Insurance: I am considered an out-of-network provider, and my services are covered by most insurance companies as such. I will provide a monthly statement called a "superbill" that you may submit to your insurance company for reimbursement. I suggest asking your insurance company the following questions to determine your coverage.
Does my plan cover mental health services (e.g., individual psychotherapy, CPT code 90834; couples/family therapy CPT code 90847)?
If so, what percentage of the cost is covered for out-of-network providers?
Is there a session limit? How many sessions are covered per calendar year?
Is precertification required?
Do I have a deductible? How much is it? Has it been met?
Payment: Made directly to me and expected at the time of service. I accept cash, check, or credit card.
